Venipuncture Order of Draw

NCCLS has recommended specific order of draw for multiple tube collections to prevent cross contamination of additives. Governing policies of each health care facility differ regarding the order of draw for vacutainer tubes. Phlebotomy standards applying to full and part-time phlebotomists, medical assistants, or other health care providers and personnel with blood collection responsibilities are set by The Clinical and Laboratory Standards Institute (CLSI)
The order of draw which is followed generally is:
First -Blood culture tubes are drawn first to obtain sterile specimens for microbiology
Second -Tubes with no additives (plain tubes) are drawn, tubes needed for coagulation studies (sodium citrate or heparin) are drawn next, if both these tubes are drawn together. A single light blue top tube may be drawn if only a routine coagulation assay is ordered.
Third - red top tube with no additives
Fourth -next draw is the tubes with other additives, gel separator tubes first, then EDTA, then oxalates and fluorides.
SST gel separator and clot activator tubes
Sodium Heparin (dark green top)
Lithium heparin anticoagulant and gel separator PST tubes (light green top)
EDTA (lavender top)
Acid citrate dextrose ACD tubes (pale yellow top)
Glucose determination tubes containing Oxalate/fluoride (light gray top)
If blood drawing is slow and difficult, there can be additive contamination from tube to tube. The potassium salt of EDTA may falsely elevate the potassium determination, if EDTA tube is collected prior to heparin tube for electrolyte analysis.
Serum separator tubes
Use of incorrect of additive or wrong amount of additive can cause adverse effects. The substance being measured may react with the additive or additive may be chemically the same as the substance being measured. Some cellular constituents may be altered or enzyme reactions may be inhibited by the additives.
Proper handling of the blood specimens after collection is therefore very important. Standards for handling blood specimens after collection by venipuncture are published by NCCLS. Serum separator tubes are used to process the clotting of blood and obtain serum. The interior of the serum separator tube is coated with clot activator (silica particles) to accelerate clotting. A barrier gel is present at the tube bottom, which will form a barrier separating serum from fibrin and cells during centrifugation. Aerosol formation is prevented since these tubes do not have to be unstoppered before centrifugation. A higher serum yield and shorter processing time along with safety is obtained with SST tubes.
Processing blood for serum
Chemistry determinations for sodium, potassium, calcium, phosphorous, acid and alkaline phosphatase, cholesterol, uric acid, and liver function tests etc. require serum. Tests in serology are also done on serum.
All specimens should be handled by the standard safety precautions since should be used considered infectious. Quick centrifugation of tube must be done to separate serum or plasma form blood cells. Potassium oxalate used as an anticoagulant leads to shrinkage of the red blood cells and intracellular water diffusion into the plasma.
Labeling with patient’s name, DOB, time of extraction and other required fields by the phlebotomist is done for the proper management of specimens, and this is ensured by phlebotomist certification. Phlebotomist’s knowledge and skills in obtaining incident reports, proper storage and preservation of samples and correct distribution to required locations is facilitated by phlebotomy training schools. In this acoustic guitar lesson let’s talk about how you should structure your practice routine.
Once you have allocated 30 minutes a day for you practice time you need to know how to divide that time up to get the best results from your efforts. If you just mess around for 30 minutes you don’t actually learn anything new and it’s really just wasted time. Now don’t get me wrong! Noodling and messing around on the guitar could be a lot of fun and there should be some of that everyday but you must realize that it’s playing guitar and not practicing.
We are all different and you might want to make a few changes but the basic concept is the same. Focus on the foundations every day.
1. Chords will be you first focus and you’ll spend 5 minutes learning and practicing 2 or 3 chords until you have the down and can change between them effortlessly.
2. Rhythms will be the next exercise and the aim here is to get a very relaxed and learn a few patterns every day. Don’t work on more than 3 patterns each day.
3. Ten minutes working on songs. You should have some chords and rhythms down and now you can use them to play some easy guitar songs.
4. Spend 3 minutes on some scales and chromatic exercises. The aim is to improve your two hands synchronization.
5. Ear Training is our second last stop. It’s absolutely important that you develop your listening skills and you need to start with some basic chord identification. Try and tell the difference between Major and Minor chords and then introduce dominant 7 chords. These 3 chord types account for 80% of all music we listen to.
6. The last few minutes has been reserved for you to play anything you like. Make sure it’s something you enjoy and never put the guitar down in a frustrated state.
